CHF Companion

Smart Home Monitoring for Heart Failure Patients

CHF Companion is an individual Computer Science Capstone Project exploring the design and development of a mobile health application for Android and iOS that supports home monitoring of patients living with congestive heart failure.

The application combines automated weight and blood pressure collection, longitudinal health tracking, an observation diary, and structured reporting for healthcare consultations.

The Challenge

Managing Heart Failure at Home Is Difficult

Heart failure requires consistent monitoring between healthcare visits. Weight and blood pressure measurements, together with information about symptoms and daily observations, can provide important context for understanding changes in a patient's condition.

01

Patient

Daily measurements, symptoms, and observations.

02

Between Visits

Information may be kept in paper notes or fragmented records.

03

Clinician

Limited longitudinal context may be available during consultation.

Inconsistent tracking and fragmented information can make it difficult to understand how a patient's condition changes over time.

The project explores how technology can make daily monitoring more structured, connected, and accessible.

Project Goal

Designing a Better Approach to Home Monitoring

The goal of CHF Companion is to design and develop a mobile application that enables patients with congestive heart failure to monitor weight and blood pressure, automatically synchronize measurements from compatible smart devices, visualize long-term trends, maintain an observation diary, and generate structured reports for healthcare professionals.

✓ Automatic measurement collection
✓ Historical trend visualization
✓ Observation diary
✓ Structured health reporting

The Solution

CHF Companion

CHF Companion brings measurements, observations, historical trends, and reporting together in a single mobile application designed to support structured home monitoring.

Automatic Data Collection

Weight and blood pressure measurements are synchronized from compatible smart medical devices through vendor integrations.

Longitudinal Trends

Historical measurements are organized and visualized to help users understand changes over time.

Observation Diary

Users can record contextual observations alongside objective measurements.

Structured Reports

Measurement history, charts, statistics, and observations can be compiled into reports for healthcare consultations.

Caregiver Support

The application supports family caregivers who assist patients with monitoring and reviewing health information.

Who We Designed For

Supporting Patients and the People Who Help Them

CHF Companion is designed for two primary user groups who work together to support consistent home monitoring.

Independent Patients

Older adults living independently with heart failure who need a simple and accessible way to review their health information at home.

Primary needs: automatic measurement collection, clear trends, simple navigation, and easy access to health history.

Family Caregivers

Family members or patient assistants who support daily monitoring when a patient needs additional help using the application.

Primary needs: access to monitoring information, straightforward workflows, and a clear view of measurements and observations.

Technology Stack

Technologies Behind CHF Companion

CHF Companion uses C# and .NET 10 across its application components, with .NET MAUI for Android and iOS, Blazor WebAssembly for the Web Portal, and ASP.NET Core for backend and integration services. Both clients communicate exclusively with the Backend REST API, while a separate Withings integration service manages device authorization and measurement synchronization.

Project Status

From Implementation to User Testing

The core CHF Companion functionality has been implemented across the Android and iOS applications, Web Portal, backend services, and Withings integration. The cloud environment is deployed, and the project is now focused on user testing, regression validation, and preparation for the final Capstone release.

System Design

Completed

System architecture, service interfaces, and data models are documented and maintained as the solution evolves.

Mobile Application

Available for Testing - Android & iOS

The Android and iOS applications include measurement trends, an observation diary, PDF reports, patient profiles, account management, and connected devices. The iOS version is available for testing through TestFlight.

Web Portal

Live / Available for Testing

The browser-based portal provides access to the same core functionality through the shared Backend REST API.

Backend & Integrations

Implemented

Backend services and Withings integration are implemented, including email verification, OAuth authorization, token refresh, webhooks, and periodic synchronization.

Cloud Deployment

Completed

The Backend API, Withings service, databases, project website, and Web Portal are deployed on Microsoft Azure.

Testing & Validation

In Progress

Automated backend service and API tests are implemented. User evaluation, regression testing, and final project validation remain the current focus.
