CHF Companion

Privacy Policy

Effective date: September 5, 2026

CHF Companion helps users collect and review weight and blood pressure information. It is not a medical device, does not provide a diagnosis, and is not a substitute for professional medical advice or emergency care.

This Privacy Policy explains how CHF Companion, an individual project developed by Anna Koriakovskaia ("CHF Companion," "we," "us," or "our"), collects, uses, stores, and shares personal information when you use the CHF Companion mobile application, related services, and website (collectively, the "Services").

1. Information We Collect

Depending on how you use the Services, we may collect:

  • Account information: your name, email address, authentication credentials, and account identifiers.
  • Patient profile information: information needed to associate measurements and diary entries with the correct patient profile.
  • Health and wellness information: weight, blood pressure, measurement dates and times, and observation diary entries.
  • Connected-device information: identifiers, authorization tokens, and connection status needed to retrieve measurements from supported device providers such as Withings.
  • Preferences: settings such as your preferred weight unit.
  • Technical and security information: IP address, device or application information, timestamps, diagnostic logs, and security events.
  • Communications: information you provide when you contact us for support or submit a privacy request.

CHF Companion does not use your device microphone, camera, precise location, contacts, or advertising identifier unless a future feature clearly requests permission and this Policy is updated before that collection begins.

2. How We Collect Information

We collect information directly from you; from a person authorized to assist you, such as a family caregiver or patient assistant; from a connected-device provider when you authorize it to share measurements with CHF Companion; and automatically through operational, diagnostic, and security records.

3. How We Use Information

We use personal information only as reasonably necessary to:

  • create, authenticate, and protect your account;
  • retrieve, store, organize, and display your measurements;
  • provide charts, trends, diary features, and downloadable reports;
  • maintain connected-device integrations;
  • send service-related account and security communications;
  • operate, troubleshoot, secure, and improve the Services;
  • respond to support, access, correction, and deletion requests; and
  • comply with applicable law and protect users, the Services, and others.

We do not use health information for advertising, marketing profiles, credit or insurance decisions, or automated medical decisions.

4. Legal Bases Where Applicable

Where data-protection law requires a legal basis, we process information as necessary to provide the Services you request, with your consent where required, to comply with legal obligations, and for legitimate interests such as protecting and maintaining the Services. You may withdraw consent at any time, but withdrawal does not affect processing already completed and may prevent some features from working.

5. How We Share Information

We do not sell personal information or share it for targeted advertising. We may disclose information only to service providers that host, secure, monitor, or support the Services, including Microsoft Azure; connected-device providers such as Withings when necessary for an integration you authorize; people you choose when you share a report; authorities when required by law; or a successor organization in connection with a transfer of the Services subject to appropriate safeguards.

6. Connected Devices and Withings

If you connect a Withings account, you authorize CHF Companion to receive the supported weight and blood pressure data associated with that connection. CHF Companion does not receive your Withings password. Disconnecting stops future retrieval but does not automatically delete measurements already stored in CHF Companion.

7. Data Retention

We retain account and health information while your account is active and only as long afterward as reasonably necessary to provide the Services, comply with legal obligations, resolve disputes, maintain security, and enforce agreements. Backup copies and security logs may remain for a limited period before routine deletion or anonymization.

8. Security

We use administrative, technical, and organizational safeguards designed to protect personal information, including authenticated access, authorization controls, encryption in transit, and protections provided by our hosting environment. No method of storage or transmission is completely secure.

9. Your Choices and Privacy Rights

Subject to applicable law, you may have the right to access, correct, receive a copy of, restrict or object to the processing of, or request deletion of your personal information. You may also withdraw consent and complain to the competent data-protection authority.

To exercise a privacy right or request account deletion, email chfcompanion.app@gmail.com from the address associated with your account. Use the subject line CHF Companion Privacy Request or CHF Companion Account Deletion. We may need to verify your identity before completing the request.

10. International Data Transfers

Your information may be processed in a country different from the country where you live. Where required, we use appropriate safeguards for international transfers and handle information in accordance with applicable data-protection law.

11. Children's Privacy

The Services are not directed to children under 13, or under the higher minimum age required in their jurisdiction. We do not knowingly collect personal information from children without legally valid authorization.

12. Third-Party Services and Links

The Services may contain links to or integrations with third-party services. Their privacy practices are governed by their own policies, which you should review before authorizing a connection.

13. Changes to This Policy

We may update this Privacy Policy when the Services, our practices, or legal requirements change. We will post the revised Policy on this page, change the effective date, and provide additional notice when required by law.
